OUTREACH SERVICES

Adult Addiction Outreach Counsellor

CASON provides Outreach services to individuals in the community. The Addiction Outreach Addiction Counsellor will provide assessment, treatment and case management services to individuals in the community where substance use is the primary concern. The formats of service include individual counselling, community-based outreach, advocacy and referral. The Youth Outreach Addiction Counsellors will provide connections to other community programs and agencies to facilitate contacts and services that will support the individual in working towards their goals as well as improving their social determinants of health.

Home for Good

In collaboration with Gateway, YWCA and Southridge, Home for Good is a supportive housing program for homeless clients with complex needs who require multiple supports. This program offers services to clients including Occupation Therapy, addictions support, ACT team support, and mental health case management where applicable. This program is designed for vulnerable chronically homeless clients. The Home for Good staff meets clients in the community and works collaboratively with clients to help them remain housed as well as work towards their substance use goals.

Indigenous Services

In partnership with the Niagara Region Native Center and the Fort Erie Friendship Native Center, an addiction counsellor is onsite between these locations during the week. The counsellor will provide supportive counselling, family and group work, case management as well as referrals to Addiction treatment facilitates or traditional native healing lodges. Services will be provided using a blend of Western clinical approaches with a unique blending of more traditional Indigenous methodologies including the use of Tribal Elders, Ceremonies and other Indigenous community supports. The counsellor with work in collaboration with the Justice Program, the Native Inmate Liaison Officer (NILO), Indigenous Criminal Court and the Mental Health Wellness Program worker.

Mobile Mental Health & Addictions

In partnership with Community Mental Health and Addictions Services (CAMHS) of Haldimand and Norfolk and Pathstone Mental Health, the mobile mental health and addictions program focuses on improving access to mental health and addictions care for people living in remote, rural and underserved communities across the Haldimand, Norfolk and Niagara Region. The counsellor in this role provides individuals who have identified personal issues with substance use and/or gambling/gaming behaviour and/or mental health. Family members who have loved ones presenting with the above concerns can also connect with the counsellor regardless if their loved one is connected to CASON or not. Staff screen and assess these issues, co-develop a treatment plan, refer to other services as required and conduct individual counselling sessions. These services are offered throughout the Niagara Region in satellite offices in the following cities: Thorold, Fonthill, Crystal Beach, Virgil & Wainfleet.

Niagara Safer Supply

Niagara Safer Supply is geared to individuals at high-risk of overdose or death due to the toxic unregulated supply of opiates in Niagara. This program works with a wrap-around support system to ensure the individuals in most need in the community, are able to access pharmaceutical interventions to prevent opiate overdoses and deaths in our region. Within our team are two peer support workers. The other wrap-around supports include; Prescribers and Practitioners, REACH Niagara, Emergency Medical Services (EMS) Paramedics, Niagara Assertive Street Outreach/ Homelessness Services, and Positive Living Niagara.

Ontario Works

CASON provides (OW) services to individuals who have identified personal issues with substance use and/or gambling/gaming behaviour. We screen and assess these issues, co-develop a treatment plan, refer to other services as required and conduct individual counselling sessions. Clients in this program set specific goals around their substance use and/or gambling/gaming behaviour and are motivated to make improvements in their lifestyle. These services are offered throughout the Niagara Region in OW offices.

Safe Beds Partnership

CASON in partnership with the Canadian Mental Health Association (CMHA) – Niagara provides a harm reduction approach to the delivery of addiction, mental health and concurrent disorders services. We screen and assess these issues and refer to other services as required and conduct brief counselling sessions. These services are predominantly provided at the CMHA Niagara’s Residential Crisis Bed program – Safe Beds.

Start me up Niagara (SMUN) & Consumption & Treatment Services (CTS)

In the community, CASON provides services at Start Me Up Niagara (SMUN) and Consumption & Treatment Services (CTS). Clients in these settings have presenting issues related to their substance use/gambling/gaming as well as other social determinants of health. At SMUN and CTS addiction counsellor is onsite providing but not limited to, one to one counselling sessions, NARCAN use, referral and resource information as well as timely connections to primary care, treatment, health and social services.
